NEW HAVEN – Registration for New Haven Public School grades one through twelve begins July 1, 2009.  Registration must be done in person at Gateway Center, 54 Meadow Street, New Haven.  The Registration office is located on the first floor. The office hours are Monday – Friday from 8:30am until 4:30pm.  If you have questions you can contact the office at 203-946-8501or 203-946-6641.
 
Parents wishing to register a student will need to provide the following documents: Child’s birth certificate or passport and proof of residency in New Haven – i.e. current utility bill (gas, phone, cable or electric) with your name and/or lease or mortgage papers.  PLEASE NOTE; drivers license, cell phone bill, credit card bill will NOT be accepted.   If you are not a resident of New Haven, you must establish residency prior to registering your child.  If you are residing with a relative or friend, you will need a notarized letter stating that you and your child(ren) live with that person along with one of their current utility bills.
 
If you have guardianship of the child(ren) through DCF you will need to present a 603 Form or letter stating you are the guardian.  If you have guardianship through the court, you will need documents stating you have legal guardianship or an Affidavit or Power of Attorney will also be accepted.
 
All children registering for school will need a current physical exam in the State of Connecticut.
