To register for school, you will need the following documents to register your child:

• Child's Birth Certificate or Passport (child should be 5 years old on or before January 1st, 2009)
• Child's Social Security Number (Optional)
• Child will need a current physical exam in the State of Connecticut (to be taken to the school after child has been registered)
• Proof of residency in New Haven (must establish residency prior to registering for school):
  • Current utility bill with your name on it (gas, phone, cable, or electric) and/or lease or mortgage papers to verify your address.
  • Drivers license, cell phone bill, credit card bill, will NOT be accepted.
  • If you are residing with a relative or friend, you will need a notarized letter stating that you and your child(ren) live with that person along with one of their current utility bills.
• If you have guardianship of the child(ren) through:
  • DCF: You will need a 603 Form or letter stating you are the guardian.
  • COURT: You will need documents stating you have legal guardianship; or we will also accept an Affidavit or Power of Attorney.

Los siguientes documentos son necesarios para matricular a su niño/a:

• Certificado de Nacimiento o Pasaporte del niño/a (niño/a debe tener 5 años antes o para el 1 de enero del 2009)
• Numero de Seguro Social (Opcional)
• El niño/a necesitará un examen fisico en el estado de Connecticut (para llevar a la escuela despues de la matricula)
• Prueba de residencia en New Haven: (Deben establecer residencia antes de matricular su ni–o/a.)
  • Recibo de utilidad (luz, gas, teléfono, gas o de agua), un contrato (lease) del apartamento o la hipoteca de la casa con su nombre para verificar su direccion. Licensia de conducir, recibo de celular o tarjeta de credito NO serán aceptados.
  • Si usted vive con un familiar, usted necesitará una carta notarizada expresando que usted y su niño/s residen en esa direccion y traer un recibo de utilidad con el nombre de la persona con quien estan viviendo.
• Si usted tiene la custodia del niño/a por estos remedios, lo siguiente es necesario:
  • DCF (Departamento de Niños y Familias): Usted necesitará el formulario 603 o una carta de este departamento indicando que usted obtiene la custodia.
  • CORTE: Usted necesitará los documentos o un afidavit del Tribunal indicando que usted obtiene la custodia legal.
